Form D- Host Family Interview
An in-home interview with a local AFS Volunteer is an important part of the application process. This is a time for you and your family to meet with a local volunteer, learn more about the program, and ask any questions you may have about the experience.

Information included in this assessment is kept confidential and will not be seen by your AFS student.
Form E- School Information
This form gives an overview of the high school your student will attend and provides details on the type, size, curriculum, requirements and activities of the school.
This form must be completed by the school and submitted to AFS.
PAF- Placement Acceptance Form
This form verifies that the host high school will accept an exchange student. It must be signed by a school official and submitted to AFS.
