After completing the FAST application you will be required to mail FAST your tax documentation.
Required Documents:
- 2011 Federal Tax Forms
- 2011 State Tax Forms
- 2011 W-2
- All appropriate schedules (for personal business, property, etc.)
- Unemployment/disability/retirement income statements, if appropriate
